Baby constipation: causes and what to do

"Does she need to poop every day?" is one of the most common questions in the early months — and the short answer is not necessarily. Real constipation isn't defined by frequency alone, but by how it comes out: hard, in little balls, with visible straining or pain.

What constipation actually is

A baby can go several days without pooping and be perfectly fine, especially if exclusively breastfed — that's not constipation. The real signs are:

  • Hard, dry, or pellet-like stool.
  • Visible straining or crying while pooping (some normal effort in newborns doesn't count — the key is whether it's actually painful).
  • Belly that's bloated or hard to the touch.
  • Sometimes, streaks of blood from a small tear caused by hard stool.

Most common causes

  • Starting solids — the most frequent cause between 6 months and 1 year, as the liquid diet shifts to solid food.
  • Switching formula or brand.
  • Low fluid intake, especially once solids have started.
  • Fever or a recent illness, which reduces appetite and fluid intake.
  • In rare cases, an underlying medical cause — more likely if constipation has been present since birth rather than showing up later.

What helps at home

  • Belly massage, circular, clockwise, around the belly button.
  • Bicycle legs — gently pedal the baby's legs while they lie on their back.
  • More fluids if solids have already started — water between meals, without forcing an amount.
  • Prune or pear juice in a small amount (one or two tablespoons), from 6 months on — they contain sorbitol, which helps soften stool.
  • Warm bath — relaxes the area and can help things move along.

When to call the pediatrician

  • There's blood in the stool repeatedly, not just a one-off tear.
  • Several days pass without pooping and the baby is clearly uncomfortable, with a hard belly.
  • Vomiting or fever shows up alongside it.
  • Constipation has been present since birth, not something that started later.
  • You notice weight loss or the baby eating much less than before.
